Check Your Financial Aid To-Do List #

Check Your Financial Aid To-Do List to See Required Documents or Verification Items

  1. To check your Financial Aid status and required documents or verification items, visit SDCCD Student Portal.
  2. Log in using your credentials. This is the same portal where you enroll in classes.
  3. After logging in, click on "view financial aid". This will show you the current and relevant information regarding your Financial Aid.
  4. To determine which forms or verifications are needed to complete your file, and to view all notifications and emails, click on the "notifications" banner on the College Student Dashboard.
  5. If your file is complete and you want to see any Financial Aid awards you may be eligible for, select "View Your Financial Aid".

Submitting Forms & Documents #

  1. Electronic Submission of some documents PDF is preferred:

    • Some forms can be submitted online.
    • Download, fill out, and sign the necessary form from the provided links.
    • Go to the 'Submit Form Online' link.
    • On the submission page, choose 'Forms/Applications' then 'Financial Aid Form/Verification'.
    • Ensure the form you select matches the attached PDF. Attach all necessary supporting documents. 
    • SUBMIT DOCUMENTS
  1. Faxing Documents: 

    • Wait 24-48 hours for faxed documents to be processed and reflected on The Portal.
    • If faxed on Thursday afternoon, updates may not appear until the following Tuesday. Check office hours for further clarity.
    • Clearly write the student's name and CSID number on every page.
    • Always include a cover page with your Name and CSID# when sending a fax.
    • Do NOT fax, email, or mail the same document multiple times or use multiple submission methods unless explicitly told to.

  2. Do NOT Email Confidential Information:

    • Do NOT email forms to any staff in the Financial Aid Office. The email server is unencrypted and not secure.
      • DO NOT submit via email any documents that have any Social Security Numbers (SSN) on them.
      • DO NOT submit via email any sensitive data documents such as but NOT LIMITED to State Driver’s License or State Identification card, Passport Card, US Passport or Military Identification etc.
      • DO NOT submit via email picture of documents with any SSN or sensitive data.
      • DO NOT submit via email any State or Federal Tax documents.
    • Emails containing private information will be deleted. Mesa College is not liable for any damages from emailing sensitive documents.

  3. Privacy & Communication:

General Forms #

You can submit some general forms by:

  • Online: Use the "Submit Forms Online" PDF preferred option.
  • Mail: Send completed forms and any required documents (unless specified otherwise) to:

  • Fax: You can also fax the materials to 1 (619) 388 – 2824.

Campus Change of Record (COR) Request#

  • To Get Started:

    • Add SD Mesa College [001275] to your current aid year FAFSA if it's not already listed.
  • Statement Submission:

    • Provide a signed statement indicating you wish to permanently change to Mesa College. You can either hand it to the FA Office or submit the PDF form online.
    • The statement should include:
        • Your name
        • CSID#
        • The semester for the desired change
        • The date you added Mesa to your FAFSA
        • Your true handwritten signature [e-signatures, images or typed font names are not acceptable written signatures for the COR change request process]
  • Confirmation & Follow-up:

    • After submitting your signed COR Statement, you'll receive a confirmation number starting with SRV.
    • To check on the status, use the confirmation number and call the Financial Aid Office at 619-388-2817. Live assistance is available M-Thr 10am-12pm & 2pm-4pm, and Fri. 10am-12pm.
  • Completion Indication:

    • You'll know your COR has been changed when "MESA COLLEGE" appears on your student portal welcome screen.
  • Warning:

    • Changing COR is time-sensitive.
    • If you've already received federal/state aid at CITY or MIRAMAR College for the year, you might not be allowed to change your COR.
    • Your COR must align with the SDCCD Campus of your academic plan.
    • Once you change your COR to Mesa, it's irreversible for the rest of the academic year (Fall, Spring, Summer).

Password Request Form for Financial Aid Students #

  1. Download Password Form

  3. Sign the form then
  4. Submit Form Online
    Send your request form with a visible copy [FRONT and BACK] of your valid Government Photo ID showing your signature (excluding Military ID). 

Third Party Authorization-Release of Student Records #

The Family Educational Rights and Privacy Act (FERPA) ensures the privacy of students' educational records, including both financial and academic information. Without the student's explicit written consent, FERPA typically restricts the sharing of this information. Hence, for any information related to your financial aid to be released, we require your authorization and password.

Only the student is allowed to complete this form. To get the Release form approved, students must also submit the Password Request Form for Financial Aid Students along with a clear photocopy of a valid photo ID.

  1. Download and fill out the Password Request form.
  2. Sign the completed form.
  3. Click "Submit Form Online" to upload the form.
    Attach clear copies of the FRONT and BACK of a valid government-issued photo ID displaying your signature (Military ID is not accepted).
  4. Submit the Third Party Authorization-Release of Student Records form

As of October 1, 2022, You can submit your 2023-2024 FAFSA and renewal applications using 2021 income information. Please be aware that the 2023-2024  application is applicable to the Fall 2023, Spring 2024, & Summer 2024 semesters.  You are encouraged to apply early.  If you apply early, you should expect to receive your first response from SD Mesa College Financial Aid approximately April-May 2023.  In the interim, you may receive automated notices indicating the application was submitted successfully, or that you may need to make necessary corrections before the school can download and process the FAFSA

 - Be Sure your Name, Date of Birth and SSN are correctly entered on your admissions record  and match your Social Security Card and FAFSA or SD Mesa College will not be able to receive your FAFSA.

  • Verification of Identity and Statement of Education (2023-24). PLEASE NOTE:  This form must be submitted in person.  It can be printed, completed then signed in blue ink in the presence of a Financial Aid Office staff member, including orignal [no copies] [FRONT and BACK] of your valid verified government issued Photo Identification (ID), such as, but not limited to, a driver’s license, other state-issued ID, or passport (excluding Military ID). Please DO NOT mail, email, fax or upload this form to the Student Support Desk document system.

VETERANS and ACTIVE MILITARY
